I really struggled to find a web form with 6 types of inputs, most have 4 that I found. This is one from my work Activate your Shop

Collect data from potential customers to start a webstore.

Team Uniforms
Document how many validation errors and/or warnings may exist on each of the two pages you validate.
On "Active your Shop" there was 13 Warnings and 42 Errors and "Team Uniforms" there was 10 warnings and 25 Errors.
How would you rate the site's design, on a scale of 1(horrible) to 10 (fantastic)? Why??
8, I love the flow of the site and the vibrant photos. I also like how each page is broken into sections with the two column layout fliping in each section.
How would you rate the site's usability on a scale of 1 (horrible) to 10 (fantastic)? Why?
6, usability is a bit clunky, lots of scrolling. Having the top nav bar stay present on the screen would help a lot.
